Working with Optimise BP will help you to control your blood pressure in lots of ways:
Easily monitor your blood pressure with Optimise BP. You can see your last BP reading, your target BP, and your average BP over the last four weeks. This helps you stay in control.
Optimise BP provides a clear outline of the next couple of actions you need to take. This helps you plan and prepare.
Keep track of the BP medication you're taking and receive clear instructions on when and how to take your blood pressure, and when to arrange for blood tests.
If things aren't going to plan, Optimise BP will alert you and your practice. You can always call the practice if you need help or contact Optimise BP directly – we will respond promptly.
If your GP practice has signed up to use Optimise BP, here's how we help:
Your GP practice may ask you to record your BP, helping to find and manage hidden high blood pressure.
Optimise BP helps you and your practice lower your BP by carrying out your clinicians instructions. The app saves time and reduces the need for appointments.
You can record your BP using your own machine, at the GP practice, or in a pharmacy. Your GP can then see these readings and add them to your GP record.

When invited to do so by your Practice, you can download our App from both the Apple App Store & Google Play Store. Please note this will only work with an Auth Code provided by your Clinician, for Self Registration you need to follow a link or QR code provided by your Practice.
Optimise BP has built-in safety features to alert you and your practice if any problems arise. You can contact your practice at any time if you have concerns.
